One of the most significant deliverables for the NFRMPO’s Mobility Program is the Coordinated Public Transit/Human Service Transportation Plan (Coordinated Plan). The Coordinated Plan aims to identify transportation issues, service overlaps, and gaps affecting vulnerable populations, including older adults, individuals with disabilities, and low-income individuals, within the urbanized portions of Larimer and Weld counties. It also outlines goals and strategies to address these identified issues.
As a recipient of FTA §5310 funds, the NFRMPO updates the Coordinated Plan every four years with guidance from the Larimer and Weld County Mobility Committees, local stakeholders, transportation providers, and other public members. The need for improved transportation for older adults and individuals with disabilities has been known. It remains an issue in large and small communities within the NFRMPO region. Current programs and strategies are insufficient to meet this growing demand and require additional support.
The Coordinated Plan is a forward-looking initiative that brings together representatives from human services and transit agencies to establish strategies, goals, and objectives for the three Mobility Committees over the next four years. Human service agencies provide transportation to older adults, individuals with disabilities, and low-income individuals. By uniting these groups with transit agencies, we can ensure each agency is better equipped to serve those in need and build a brighter future for all.
SAFETEA-LU and subsequent transportation legislation require Coordinated Plans to:
- Identify the transportation needs of individuals with disabilities, older adults, and people with low incomes;
- Provide strategies for meeting those needs; and
- Prioritize transportation services for funding and implementation.
The Coordinated Plan is updated every four years, and the current 2025 Coordinated Plan was adopted by the NFRMPO Planning Council on December 4, 2025, with support from the Larimer County and Weld County Mobility Committees and the joint Northern Colorado Mobility Committee. The 2025 Coordinated Plan resulted from an inclusive planning process involving older adults, individuals with disabilities, low-income people, public and private transportation providers, and other stakeholders. The NFRMPO completes the Coordinated Plan on behalf of the three large transit agencies in the region (Transfort, GET, COLT).
The Coordinated Plan identifies the transportation and mobility needs of individuals with disabilities, older adults, and people with low incomes and consists of three distinct sections:
- Analysis of existing conditions, including demographics and providers;
- Analyze transportation needs for individuals with disabilities and older adults, including gaps in service; and
- Strategies, activities, and/or projects to address the identified gaps between services and needs.
The Coordinated Plan builds upon the mobility management efforts outlined in previous Coordinated Plans. Examples of mobility management include transportation brokerages, coordinated transportation services, building and supporting local partnerships, travel training, trip planning, one-call/one-click centers, and investing in technologies such as intelligent transportation systems (ITS). Based on this information, the NFRMPO recommends a prioritized list of implementation strategies that account for resource availability, time constraints, and feasibility. This list of strategies will be in effect from December 2025 to December 2029.

Mobility Action Plan
To further the goals and strategies identified in the Coordinated Plan, the Larimer County and Weld County Mobility Committees, along with the joint Northern Colorado Mobility Committee, created and adopted the first Regional Mobility Action Plan (MAP) in early 2022.
For the 2025 Coordinated Plan, the Mobility Staff is working on updating the Regional Mobility Action Plan to be adopted by the Larimer County and Weld County Mobility Committees, along with the joint Northern Colorado Mobility Committee, to adopt the updated Regional Mobility Action Plan (MAP) in early 2026.
